Avoid assumption. 

When writing. Particularly your second or even third book in a series. Never assume that the reader has the faintest idea what someone looks like or what the place you forgot to describe fully looks like. 

It doesn’t hurt to repeat a little information. 
Try not to be assumptive i.e assume that the audience knows there’s a table in the room when you write. ‘He crossed to the table.’

Far better to write. ‘There was a large, oak table in the room, and he found himself drawn to it.’

So the lesson. Don’t assume.
